Based in the southwestern Colorado town of Pagosa Springs, this 22,850 s.f. two-story medical center is a small, critical access point serving a far reach that extends into New Mexico. Bolstered by support from the nearby New Mexico Apache tribe, this clinic utilizes team care to serve a community facing physical, social and income-level challenges.
This is the first phase for the program of this outpatient clinic and lab, which houses both primary care providers on the first level and rotating specialists on the second, and is designed to promote team-based collaborative care via a central pod system located on each level. This approach to population health is based on the concept of the patient-centered medical home, where care teams take a holistic approach to include education, coordinated dental, behavioral health, and group visits.
